Adoption is outrunning governance — most enterprise agents still run with no dedicated identity or accountability controls, which is exactly the gap regulators are now racing to close.

1 Agentic AI
Autonomous, tool-using AI is moving fast in the enterprise. The safe-deployment question for an institution is less "can it act" and more "can you prove what it did, and why."
Enterprise agentic AI has a large adoption-versus-production gap: roughly four in five enterprises have adopted AI agents in some form, but only around one in nine runs them fully in production — the largest deployment backlog seen in enterprise technology. NIST's Center for AI Standards and Innovation opened its AI Agent Standards Initiative in February 2026 precisely because most agents are still run as generic service accounts with no dedicated identity, authorisation or accountability controls.
NHS England and Optellum have launched a single, end-to-end AI-supported lung cancer diagnostic pathway — automatically coordinating triage, imaging review and referral steps that previously depended on manual handoffs between separate teams.
None of the NIST AI Risk Management Framework, ISO/IEC 42001 or the EU AI Act yet name "agentic" systems directly. NIST's own concept paper this year — on identifying, authenticating and authorising software and AI agents — is the clearest signal yet that formal standards are still being written, not already in place.
Before letting an agent act autonomously on a real system, give it one narrow, reversible task with a human sign-off step. Only widen its scope once you can produce a clean audit trail of what it did and why — that trail is what governance teams will ask for first.
2 Robotics AI
Physical AI is entering hospitals for surgery and sampling. Deploying it safely means treating it as a regulated medical device from day one, not a lab demonstration.
Medtronic unveiled Touch Surgery™ Aide at the Society of Robotic Surgery's 2026 Annual Meeting (21–26 July) — an AI-native surgical computing platform combining computer vision and real-time inference so multiple AI applications can run simultaneously during a live operation, across an ecosystem already used in more than 1,500 operating rooms worldwide.
NHS England's AI-and-robot lung cancer pilot at Guy's and St Thomas' uses robotic bronchoscopy — an ultra-thin, shape-sensing tube — to biopsy nodules as small as 6mm, previously too small and hard to reach with standard techniques. Full rollout began in 2026, with planned expansion to King's College Hospital and Lewisham and Greenwich NHS Trusts.
Surgical robots are regulated as medical devices under the UK Medical Device Regulations, requiring risk-based conformity assessment by a notified body before use — the same regulatory track MHRA continues to update through its ongoing Software and AI as a Medical Device guidance.
Pick the one physical, repetitive task in your pathway with the clearest error signal — a count, a time, a defect rate — and pilot robotics-assist there first. It's far easier to build a safety case when you can measure the failure mode directly.
3 Ambient AI
Ambient scribing is now mainstream across the NHS. The safe-deployment bar is consent, transparency and a human sign-off — not just transcription accuracy.
Ambient AI scribing has scaled fast in 2026: one large NHS deployment on its own will put ambient scribing in front of 10,000 clinicians in its first year, scaling to 20,000 within four years, as adoption accelerates across primary and secondary care.
NHS England published new national guidance on 31 March 2026 for using AI-enabled ambient scribing products in health and care settings, with further support — business case templates, example hazard logs, safety cases — being published through 2026 and 2027 to back the rollout.
NHS England's guidance is explicit that consent isn't required to use an ambient scribe in individual care — but transparency is: clinicians must tell patients when it's running and give them the chance to object. A June 2026 review has since called for stronger national oversight as the rollout scales.
Before switching on an ambient scribe, write down — one sentence each — how a patient is told it's running, how they can decline it, and who reviews the note before it's signed off. If you can't answer all three today, you're not ready to deploy.
4 Clinical AI
Diagnostic AI is being funded and rolled out nationally. The harder work for a real institution is proving it performs fairly across every patient group it will actually see.
A near-£30m government package announced in mid-2026 is accelerating NHS AI diagnostics: £20m to extend AI-assisted chest X-ray analysis — already used by roughly half of NHS trusts, and credited with helping over 4 million patients toward faster lung cancer diagnosis or clearance — to every trust by 2029, plus £8.1m from the NIHR to trial six further AI and digital technologies across 13 sites.
Skin Analytics' DERM AI skin cancer triage tool holds NICE's conditional approval for continued NHS use for three years while further real-world evidence is gathered, and is already supporting faster diagnosis in community care across Merseyside.
A widely cited Nature Human Behaviour study found clinicians who repeatedly worked with a biased AI system became measurably more biased themselves over time — a feedback loop not seen in human-to-human interaction, and now a central argument for mandatory subgroup performance reporting in clinical AI governance.
Ask your clinical AI vendor for subgroup performance data — not just an overall accuracy figure — before a single patient is triaged by the tool. If they can't break it down by the groups your service actually sees, that's your answer.
